First physical then mental. For me I have been fortunate not to have any physical “problems” with being locked. However my situation is that I am not locked for long periods of time…I am denied for long periods of time though. It is a mental game and how you approach it. For me, being chaste is a gift to my wife and her lover. I don’t know if you are being “forced” into chastity. Again each couple is different. You will get “blue balls” maybe some discomfort after awhile, but that goes away with time. For me that is around 4 weeks of no orgasm. I don’t know what your time frame is planned on. Don’t be concerned about the “size reduction”, if that happens, it will not be a huge amount and besides…your wife probably is already being taken care of by her other man.
Now mentally…can be hard as hell. You have to go into this with an open mind and remember the end result. This is not always about you or me…it is about our wives and their other partner. I know it is hard to understand…hope this helps

I can’t say I noticed any prolonged issues of size reduction, etc… I will say that getting the right fit is crucial, especially in the base ring. Too big and you risk a testicle being pulled through (ouch), too tight and even though it might feel comfortable at rest or even play, waking in the middle of the night with constriction pain from nocturnal erection is, memorable, and not conducive to good health.
Also, good hygiene is critical (no woman likes rotten penis stink) which is why we only use stainless steel cages (no tubes).
We’re still struggling to find “just the right one”. Jail-bird from Mature Metal has been the best yet, but not as secure as we wanted (can pull out). Got an PA and tried to adapt a few mods, but nothing was “perfect”. Recently got a new Modular cage from Rigid Chastity, but got my measurements off (ouch) and though friendly, they seem to be overworked and very slow to respond to customer service requests (plus, they are in Poland, a long way from us).

There really should not be any issues from prolonged use, the only medical downside is long periods between ejaculation can cause a build up of prostate acids which have been shown to contribute to prostate cancer, but research still being done.
Otherwise there is no reduction in size or ability to attain erections, it can make you a bit quick on the trigger though when allowed sex.
Proper fit is tantamount and many do not realize that. It may take 3-4 times to find the right size, so find a supplier who doesn’t mind a return or two, also you need to supply real time measurements. Metal is easier on the balls than plastic, also easier to keep clean
